We are a leader in designing innovative primary care investment payment and delivery models that empower states to achieve the Quadruple Aim of improved health outcomes, patient experience, and health equity while lowering costs. Collaborating with payers, providers, hospital systems, actuaries, and national experts, FHC has helped more than half a dozen states develop multi-year standards and actionable targets for increasing primary and behavioral health investment, limiting price growth, and expanding the adoption of alternative payment models. Recent Publications include:

Delaware Health Care Affordability Standards: An Integrated Approach to Improve Access, Quality, and Value, which includes plans to more than double primary care spending in the commercial fully-insured market by 2025.

Investing in Behavioral Health Care: Lessons from State-Based Efforts (February 2023) includes a comprehensive list of behavioral health service codes to help states and nonprofits create standard comparisons.

Working with state healthcare leaders across the country, FHC developed a comprehensive toolkit highlighting the team’s successful efforts to implement new state-level primary care investment initiatives. This document details the opportunities and action items, offering practical guidance to ​support state leaders in advancing policies that strengthen primary care, improve health outcomes, and ensure sustainable investments in their communities.

Review of Delaware Health Plan Compliance with State and Federal Law (January 2023)

In Delaware, FHC manages the Office of Value-Based Health Care Delivery under the leadership of Delaware’s Department of Insurance. The Office is charged with developing Affordability Standards and targets for carrier investment in primary care.

Measuring Non-Claims Payments

FHC worked with states and national organizations to grow the adoption of the Expanded Non-Claims Payments Framework (Expanded Framework), which it developed with the California Department of Health Care Access and Information (HCAI) in 2023. In April, the National Association of Health Data Organizations (NAHDO) included the Expanded Framework in its national Non-Claims Payment (NCP) Data Layout to standardize the collection of non-claims payment data across the nation. Understanding impact of increased primary care investment

FHC examined the relationship between primary care quality and spending among Massachusetts provider organizations. The report “An Analysis: Massachusetts Primary Care Investment and Quality” found that as primary care investment increases as a percentage of total spending, organizations performed significantly better on standardized quality measures and had significantly lower spending on inpatient and outpatient hospital services. It also found that these provider organizations did not have significantly higher total spending. This work also was featured in a recent Health Affairs blog post.
